2. Description of Services

Code of Care provides independent aged care coordination services, including but not limited to:

• Care planning and needs assessment
• Navigation of the aged care system
• Provider selection and coordination
• Advocacy and support services

We act as coordinators and do not directly provide aged care services such as nursing, personal care, or domestic assistance.

3. Eligibility

Our services are available to individuals in Queensland, Australia, and their families or representatives. You must be at least 18 years old to engage our services on behalf of yourself or another person.

4. User Responsibilities

When using our website and services, you agree to:

• Provide accurate and complete information
• Use the website and services only for lawful purposes
• Not attempt to interfere with the proper functioning of the website
• Respect the confidentiality of any information shared during our engagement

5. Fees and Payment

Our initial consultation is provided at no cost. Ongoing coordination fees will be discussed and agreed upon before services commence. Fees may be payable from your Home Care Package or Support at Home funding where applicable.

6. Limitation of Liability

Code of Care provides coordination and advisory services only. We do not guarantee specific outcomes from aged care providers or government programs. To the maximum extent permitted by law:

• We are not liable for actions or omissions of third-party aged care providers
• We are not liable for decisions made by government agencies regarding funding or assessments
• Our liability is limited to the fees paid for our services

7. Intellectual Property

All content on this website, including text, graphics, logos, and design, is the property of Code of Care and is protected by Australian copyright law. You may not reproduce, distribute, or create derivative works without our written permission.

8. Privacy

Your use of our website and services is also governed by our Privacy Policy, which explains how we collect, use, and protect your personal information.

9. Termination

Either party may terminate the coordination relationship at any time with reasonable notice. Upon termination, we will provide you with copies of any relevant documentation and assist with transition to alternative arrangements if requested.

10. Dispute Resolution

If you have a complaint or dispute, please contact us first to seek resolution. If we cannot resolve the matter, you may seek assistance from relevant consumer protection or aged care complaints bodies.

11. Governing Law

These Terms of Service are governed by the laws of Queensland, Australia. Any disputes will be subject to the exclusive jurisdiction of the courts of Queensland.

12. Changes to Terms

We may update these Terms of Service from time to time. Continued use of our website or services after changes constitutes acceptance of the updated terms.
